Hair Procedures: PRP, Microneedling, Transplants
Office-based and surgical options, with candidacy and long-term planning placed before the procedure.
Hair procedures range from injections and microneedling to surgical transplantation. They can involve repeated visits, pain control, wounds, lasting scars, multi-week visible-healing and activity considerations, and substantial cost. PRP evidence is encouraging but protocol dependent, while transplantation redistributes finite donor follicles. Diagnosis, candidacy, provider qualifications, aftercare, future loss, and total financial commitment belong before the procedure.
“Procedure” covers interventions with very different goals, evidence, invasiveness, and permanence of physical changes. An injection series, controlled skin injury, and follicle transplantation should not be compared as if they were versions of the same service.
This category also has the highest logistical burden. Appointments, preparation, pain control, aftercare, time away from normal activities, visible healing, and access to complication management can matter as much as the procedure time itself.
Begin with diagnosis and candidacy
Before discussing a device or surgical technique, establish what process is causing the change. Procedures can be inappropriate or poorly timed when loss is unstable, inflammatory, scarring, or unexplained. Scalp disease may need treatment first. Unrealistic density goals, weak donor supply, or a pattern likely to progress can also change the plan.

Platelet-rich plasma
Platelet-rich plasma, or PRP, is prepared from a person's own blood, processed to concentrate platelets, and injected into the scalp. Platelets release signaling molecules involved in wound response and tissue communication. That mechanism does not define the clinical effect.
Systematic reviews report average improvements in hair density or thickness in selected people, but studies use different preparation methods, injection schedules, comparators, and outcome measures. PRP is not one standardized product. A consultation should address the protocol, evidence, expected outcome, discomfort, cost, maintenance, and uncertainty.
A systematic review and meta-analysis found generally favorable density and thickness results across included studies while calling for lower-bias randomized trials and standardized protocols [1]. Variability makes it hard to know whether evidence for one preparation applies to another.
Risks can include pain, bleeding, bruising, swelling, headache, infection, and vasovagal reactions. Individual medical conditions, medications, and blood or platelet issues may affect suitability. A clinician should discuss them before treatment.
Microneedling
Microneedling creates controlled punctures with needles. It has been studied alone and as an adjunct to topical treatment, but devices, needle depths, schedules, endpoints, and combination protocols vary. Office procedures and consumer rollers are not interchangeable.
Potential harms include pain, bleeding, irritation, infection, pigment change, scarring, and altered absorption of topical products. Applying a drug immediately after needling can change exposure and should not be improvised. Diagnosis, device sterility, depth, scalp condition, and aftercare belong with a qualified clinician.
Hair transplantation
Transplantation moves follicular units, typically from a donor region at the back or sides of the scalp to areas of loss. It does not manufacture new follicles. Aesthetic outcome depends on donor quality and quantity, graft handling and survival, hairline design, angle and direction, recipient-site planning, scar behavior, healing, and the surrounding hair's future.
International consensus emphasizes appropriate preoperative assessment, counseling, and postoperative care [2]. A clinical overview likewise frames transplantation as surgery whose outcome depends on diagnosis, donor supply, technique, design, execution, and postoperative management rather than graft count alone [3].
FUE and FUT
Follicular unit excision, or FUE, removes follicular units through many small punches. Follicular unit transplantation, or FUT, removes a strip of donor scalp that is divided into grafts. Both can produce natural results in skilled hands, and both create scars. Choice depends on donor characteristics, hairstyle, goals, prior surgery, and surgeon judgment.
FUE can leave many small round scars and can thin a donor area if overharvested. FUT creates a linear scar and involves sutured closure. Marketing either method as “scarless” is misleading.
Future loss still matters
No. A transplant redistributes selected follicles; it does not stop susceptible non-transplanted hair from continuing to thin. Long-term planning must consider the likely future pattern, finite donor supply, age, family history, medical treatment preferences, and the possibility of additional procedures.
Potential surgical complications include infection, bleeding, swelling, numbness, poor growth, cysts, unnatural appearance, wide or visible scars, donor depletion, shock shedding, and dissatisfaction. Risk varies with patient factors, technique, team, and aftercare.
Recovery and downtime
Downtime varies. Injections or microneedling may cause short-term tenderness, redness, swelling, or activity restrictions. Transplantation can involve visible crusting, swelling, donor and recipient healing, sleep or exercise modifications, sun protection, and grooming limits across multiple weeks. Final transplanted growth is assessed over months, not at the end of visible healing.
Recovery is not limited to whether a person can technically return to a desk. Visible healing, restrictions on sweating or exercise, careful washing, sleep positioning, sun avoidance, and the ability to wear a hat can affect when someone feels ready for ordinary work or social activity.
The result also has a separate clock. Transplanted shafts commonly shed before new growth becomes visible, and final evaluation occurs many months later. Scheduling should account for both the early healing period and the longer cosmetic transition.
Other office procedures
Clinics may offer injections, lasers, exosome or cell-derived products, combinations, and other regenerative claims. Regulatory status and clinical evidence can lag behind marketing. Ask whether the exact product and route are authorized for the stated use, whether robust human trials exist, and what is known about composition, sterility, durability, and adverse events.
An “autologous,” “natural,” or “regenerative” label does not remove risk or prove effectiveness.
How do I assess a clinic?
Verify the diagnosing and operating clinician's license, training, board certification where applicable, and personal role in each step. Ask about candidacy, alternatives, donor estimates, scar patterns, complication management, team responsibilities, standardized results, conflicts, aftercare, and who provides urgent follow-up. Avoid guarantees and pressure-based sales.
Request photographs with consistent lighting, angles, hair length, styling, and follow-up. Ask how many grafts were placed and whether the examples reflect a similar hair caliber, curl, color contrast, pattern, and donor area.
Plan the total burden, not only the appointment
Ask for an itemized estimate that includes consultation, testing, procedure fees, anesthesia or pain control, medications, aftercare, follow-up, travel, time away from work, and possible maintenance or repeat sessions. A low advertised procedure price can omit parts of the real commitment.
Clinic pricing varies widely by protocol, geography, provider, graft count, maintenance, and what the quote includes. A multi-session PRP course can become a four-figure commitment, while an extensive transplant can reach five figures. Treat website ranges as dated planning estimates, request itemized quotes, and include travel, time away, medications, aftercare, and possible repeat treatment.
For transplantation, also discuss how many grafts are proposed, what donor reserve remains afterward, what density is realistic, and how the design accounts for future loss. Travel surgery can complicate urgent follow-up, infection management, or corrective care. Cost should not be separated from access to the operating clinician and the team responsible for complications.
Recovery planning should include ordinary life. Ask when the clinician expects the patient to wash normally, exercise, sweat, sleep in a preferred position, use a hat, expose the scalp to sun, color or cut the hair, travel, and appear at work or a social event without obvious healing. The answers vary, but they should exist before consent.
Compare a procedure with non-procedural alternatives
A procedure may be appropriate when the goal is coverage or another outcome that non-surgical approaches cannot provide, or after other suitable strategies have not met the goal. It should still be compared with monitoring, an oral supplement, topical or prescription therapy, a device, cosmetic coverage, and the choice to do less.
The comparison is not only “Which produces more hair?” It is also:
- Which option preserves existing function versus redistributes coverage?
- What uncertainty remains about the diagnosis?
- What pain, visibility, recovery, and complication risk is acceptable?
- How many future sessions might be needed?
- What other approaches may still be required afterward?
- What happens if surrounding hair continues to change?
For a person early in the process with functioning follicles, an oral strategy may be easier to try, but ease does not establish preservation or treatment efficacy. For a carefully selected person with advanced stable loss and adequate donor supply, surgery may address a different goal. Those are not contradictory conclusions.
They reflect different stages, targets, burdens, and definitions of success. A fair site should help readers see that difference before a clinic consultation or purchase decision.
Compare procedure burden honestly
Use all approaches, the comparison tool, and how we read evidence to compare not only possible benefit but also invasiveness, downtime, repeat sessions, maintenance, uncertainty, and who manages complications. Someone with functioning follicles and no urgent clinical issue may evaluate a lower-burden strategy before invasive restoration.
